My wife has celiac disease and is allergic to flax. Her meals must be gluten-free. Where in PEI can we eat safely? Just having lettuce salad on the menu does not count.

I agree that a salad is not enough, not while visiting Prince Edward Island!
I am pleased to answer your question because I am a bit of a foodie and I love my restaurants around the Island. There are many wonderful spots.
In Charlottetown, Water Prince Corner Shop offers a gluten free menu and dessert. Shaddy's is excellent, the owner has celiac so understands the requirements. The Chip Shack is also gluten free. John Brown on Victoria Row, their entire menu can be made gluten free!
In New Glasgow, the PEI Preserve Company have several gluten free options.
Darnley check out Ship to Shore.
In Kensington check out Go Fish, Family & Friends and Island Stone Pub.
In Stratford, Phinley's is very popular. Great family diner.
Ol' Boys Eatery is an amazing food truck and serve gluten free options. Quickly becoming the go to food truck!
